Shoutcasters are supposed to give countdown before each round. You have to click on pause when you hear this countdown then click on play when you see this countdown on ETTV.

pause at a point you think you know when it will happen ingame (countdown at start) if you forgot that one look for some 3 man kill or w/e the shoutcaster sometimes says the time ingame also so you can use that 2 :D

OR what i do dont listen to it because i cant hear him anyways because of the shots

A shoutcast streams delay is usually 30-45 seconds behind live, a game's TV delay can be anything from 1 to 5 minutes. With this in mind, the shoutcast will broadcast events before you see them in your game. The easiest way to sync them is by assigning global hotkeys to control the audioplayer, this way you can play/pause/stop the shoutcast ingame - and that's all you need:
When the count down begins and the match is about to get underway the shoutcaster will often either countdown 3,2,1 FIGHT, or just FIGHT. As soon as you hear FIGHT, instantly hit pause. When the TV server you are connected to catches up and you see FIGHT on your screen, unpause your player by hitting your hotkey. Throughout the match the shoutcaster may give several “sync up's” aimed at helping the viewer to synchronize the two, utilise these as you would if it was the start of the match. The shoutcaster will perhaps say the time on the clock is 10 minutes, pause at this point, and again unpause when you see 10 minutes on your game TV.
If you miss the sync and suddenly the shoutcast lags behind the live action, hit your stop-hotkey and then the play-hotkey. That way you'll turn the shoutcast live again and start the sync-process.

If you think they are useless don't listen to them. Someone who likes them will listen :D

Did you enter the ip via Media -> Open network-stream (or however it's called, -> CTRL+N)?
Also try to add 'listen.pls' to the url... e.g. http://xx.xx.xx.xx:xxxx/listen.pls
Finally maybe the shoutcast-server is just offline?
